Occasional Glans Discomfort After Orgasm

Complaint:  

For much of my adult life, occassionaly I feel a stinging sensation on the head of my penis (glans) about 10 minutes after I have an orgasm (both after sex and masterbation).  



The stinging is kind of a cold sensation and it makes me feel like I have to urinate, even when I don't really have to.  If I do try to urinate, it seems to get worse.  Urinating does not burn, it just makes the pain come back worse after I'm done urinating.



I noticed that the glans looks swollen and the rim looks especially red and inflammed.  It seems to help to squeeze the head a little.  The paid goes away after about 30-60 minutes.



History:  

I'm 31 and have had only one partner, my wife, for almost 10 years.  Before meeting my wife, I had unprotected sex with several women.  When I finally met my wife and "grew up" I got tested a couple times for STDs (swab and blood tests) and the results were negative.  I don't have any other sexual dysfunctions or pains.



I do worry sometimes, however, that there may have been something the tests didn't detect.  There were a couple of my past partners that I had heard ended up getting some kind of STD or infection, but I don't know what.  



Also, about 15 years ago, a friend who had sex with one of my former partners ended up having some kind of infection.  After I had sex with that partner, I had some kind of small discharge for a week or two, without any other symptoms, but then it went away.



My wife has only had sex with me and we have had two children.  Several years ago she had some kind of bumps between her vagina and anus.  Her doctor was worried that it was warts but then determined that it was only some kind of skin irritation.  She has always been somewhat prone to yeast infections.  Other than that she has never had any problems.



Question:

Can you tell me what is causing the pain?  Do you think it could be an STD or infection haunting me from my past?

Difficult to say without examination.  You can be tested for various STDs that may be causing your symptoms - including gonorrhea, chalmydia, herpes or syphillis.  Appropriate blood and urine tests can be arranged via your personal physician.



If infection has been appropriate evaluated for, you can consider other options, such as a urethral polyp or stricture.  A urologist can perform a cystoscopy - which would be a pretty comprehensive evaluation of the lower GU tract.



These options can be discussed with your personal physician, or in conjunction with a urology referral.



Followup with your personal physician is essential.
