Prostatitis/Phantom Pain or What!

I am 52 yrs old, I have had prostatitis on and off since "79.

In the last five years, it seems i have had an annual flareup,take Levaquin and ok. I had a Iguinal hernia repair 15 months ago and I still have a dull ache in my cord and testicle, the Urologist said it could be phantom pain.  while I was there, i complained that I am an avid sportsman and ride atv's and horses plus mowing, and when i do, i get irritation frequent urination and feels like i need to have bowel movement or like a golf ball in my rectum. He tells me to avoid this activity.  These are the things that I live to do, he does say take Advil and sitz baths, I have taken advil and it helps but does tend to give me diarrhea. He also said that i could have T.U.M.T. that it normally is for enlarged prostate but it could help my situation and if not it would not hurt anything. Also, after i had the hernia repair when i would ejaculate, i would get pain in the cord and testicle, as time goes by that has gotten better, but can still feel it. If I keep giving up things i love to do, i'll be old before my time. I also average getting up 1 to 2 times a night. I also consume no alcohol, do have two cups of coffee in morning. Chronic prostatitis can be very difficult to treat.  With almost 30 years of this disease, I would assume that much of the work has been done.  One thing to consider would be a transrectal ultrasound to evaluate for any prostatic abscesses.



There are small studies suggesting medications like finasteride and alpha-blockers on a chronic basis may be helpful.  



A small study suggested some benefits with mepartricin, which lowers estrogen levels in the prostate - more study is needed with this medication.



You can discuss these options with your personal physician, or in conjunction with a urology referral.  



Followup with your personal physician is essential.
