I have had recurrent UTI's since July 1997 and have taken 7 or 8 different antibiotics. They never go away. I have had numerous tests and am scheduled for urethral dilation this week as last ditch resort. I have also recently been prescribed mandelamine. Not sure what to do.

Your question is one that comes-up frequently.
Urethral dilatation is an old urologic practice that has not been scientifically proven. Rarely, a true urethral stricture is found. This requires calibration and probable reconstruction.
If you have had infections that do not get effectively treated (cultures during and after treatment are still positive for the same bacteria), then an IVP and cystoscopy and possibly even a urethrogram may be indicated. If your infections are really new infections, prophyllaxis with nitrofurantoin either on a continuous basis or, If related to sexual activity, on an episodic basis are reasonable ways of dealing with this.
