Tecnis CL Silicone iol and pupil width

I am 49 and when I was 45 I got cataract in the right eye. At this time I decided not to undergo surgery and wait for new iol’s technology. Now I decided for the new Tecnis CL Silicone iol but my doctor told me that, because my pupil width (5 mm), the Tecnis will not fit because they are for standard pupil’s width and I have to turn in a traditional one. I am quite resolute either to have Tecnis or no surgery at this moment and wait, but my vision is very poor even because the left eye starts having cataract too. Browsing in Internet I did not see any issue about Tecnis and pupil width. Does someone know about this matter? I apologize for my English that is not my language.

i dont know either.  the best advice i can give you is to get a second opinion.

go to www.tecnisiol.com. The aspheric IOL's like the tecnis work to provide better contrast as the pupil enlarges to 5mm. If you are out of the USA , you might be talking about Tecnis Multifocal which is different.

Thank you. I already visit the tecnis site, but could not find about. I live in USA and I know that tecnis multifocal are still in clinical trial. I can seek for them in Europe but, reading the posts in this forum, I see that generally multifocal iols can cause more problems than they can resolve and probably the ideal iol has still to come.

yo are right. Tecnology will always continue to get better. Multifocal IOL's today are gaining in interest and implantation. They are provide great results for many people. It is very important to talk to your doctor about your lifestyle, occupation and hobbies to determine what is best for you and what trade offs there are. It never hurts to talk to more than one doctor and research as you are doing so you go in with the right mindset. You should be fine for the Tecnis CL. I do not understand the comment about the pupil size and you should ask more questions to your doctor about it. It would not provide as much benefit if you had a very small pupil.

Yes. I went to my doctor yesterday. He told me that problem concerns the capsular bag diameter that in my case is bigger than the average. I was nearsighted before cataract (about 4.5 dioptries and 1.5 of astigmatism), but he is thinking to do better measure and taking more information about.  He is the thirds doctor that I see in three months. The first one told me that I had also a Keratoconus and you can imagine how depressed me such a diagnosis. At this time I was wearing contacts and this affected the topography. He did only one and came out with this. He told me also that Tecnis are just commercial and suggested a traditional spherical iol. The second one confirmed the keratoconus, telling also that with time my vision will worse and eventually I’ll face cornea’s transplant. The last one and my actual doctor instead found that I have regular astigmatism and when I told him about the Keratoconus he answered that my eyes have regular astigmatism and cataract and for the rest they are healthy and normal. The cornea’s thickness is very good too. I believe him also because I had eye’s examination two times in Italy this year and none did diagnosis of keratoconus.

He agrees with me that aspherical iols are better and, if possible, he will implant the Tecnis CL. As you can imagine, I am pretty tired to change doctor and I decided to post in this forum instead. I trust him anyway. In my opinion, ophthalmologists need time to know and be trained with these new iols and that is normal.
