Cataract Experience

I am a 56 year old diabetic and had both eyes done with multifocal iols, the right eye on October 31st followed by the left eye 2 weeks later on November 14th. The right eye had an advanced cataract but just a slight one in the left. After the first one was done I noticed a heaviness in that eye which I assumed was just the after effects from the surgery, but after a few days I noticed a shadowy arc shape at the outside edge and round the lower eye. Also a fuzzy sensation.

At the check up a week later the surgeon could find nothing wrong  - he checked my retina as well. The following week I noticed that the heavy and fuzzy sensation would go away when I wore my old reading glasses, even when just balanced on the tip of my nose and looking into the distance but not through the lenses. When I removed the glasses the sensation would come back again after a few seconds. This really amazed me and I started to think my brain was just playing tricks on me. I found that the sensation would go away if I held a piece of card over my cheek or put my hand over my cheek.

When I had my left eye done 2 days ago I mentioned this to my doctor and he said it must be just light reflected back from my cheek and will go away with time. I don’t get this at all in my left eye now I’ve had that done.

Has anyone out there experienced anything similar to this and does it go away eventually. I can live with seeing a grey shadow (I can see my nose if I look at it but it never bothers me) but it’s the heavy pressure sensation that really bothers me.

Maybe not that specific as I just think it is excessive glare, and by covering up my cheek it probably just shields the invisible light rays from my eye. I tried to describe the way my brain is processing something it cannot see but it is probably the same feeling as when a weak sun is shining at the corner of your eye. It's strange that I only get it in the right eye. After doing some web searching I think it might be "negative dysphotopsia" so if anyone has experience of that please tell me. When doctors find nothing physically wrong with the eye I think they just dismiss these symptoms as being unimportant and say it will eventually go away, but unless they follow up their patients after 6 months and 1 year then they will probably never know. But I find it very irritating.

Yes, I had this same sensation.  The reason I believe it goes away when you are wearing glasses is because you cut down on the glare from your cheek.  It was very annoying to me also.  But it did go away.  There are so many things that patients are not warned about when receiving a multifocal lens.  And if they were, I believe it would cut down on so much unnecessary anxiety.

Example:  Poor vison after surgery. After 3 months became perfect.  Went 3 months thinking it was permanent.

Enormous glare.  After many months, have very little glare now.

Debilitating psychedelic night show.  After 8 months, that is actually improving to the point that I cannot believe it.  Had I been told how many things are possible, and that time, even 8 months is necessary in some cases, it would have reduced my anxiety a great deal.   Time changes so many aberration from the multifocal lens. But in the mean time, can be a big stressor. Best wishes. You are very early on.

I am absolutely gob-smacked that you also had reflections from your cheek. I had given up hope of anyone replying to my question and your message has now given me renewed hope.

Can you describe exactly the sensations you were getting from this? Did it go away when you put your hand over your cheek? How long did it take to go away? Did it go away gradually or suddenly?

Although it doesn’t affect my vision (in fact I now have excellent vision all round and no longer need glasses and no halos either) I am feeling a lot of pressure and strain in my right eye. Also I sense a fuzziness at the corner of my eye when looking directly ahead, but when I look across to the right there is nothing there. I also see a dark shadow which I presume is my cheek or lower eye lid, but this doesn’t bother me anymore and only notice it when I put my attention on it. It feels like my eye is still affected by the anaesthetic - heavy and tired. Also it is slightly better in the evening under electric light. Did you get all these symptoms – I’m dying to hear from you again!

Tony, I had  many aberrations from my one multifocal implant. First my vision was not clear for 3 months.  After that came in, I got glare from inside lightening.  In stores, the glare was so great that I could hardly see in from of me.  I was operated on Feb 27, 06. The glare started in April, and also the reflection on my cheek.  I would put on glasses just to stop the annoyance. I still had the glare in July but it is almost gone now and no longer have the reflection on the cheek that you were referring to. I had debilitating halos and after 8 months, they are just now decreasing daily.  I am so shocked becasuse I thought it was something that I would just have to live with, after having for so many months.  Tony, the only thing I can tell you is that changes go on for a long time with a multifocal.  And in my experience, it keeps improving. It can take a long time for some.  But apparently the brain does adapt, as I was told, but after so long, no longer believed.  Just give it time.  Many changes take place.  Even in my 9th month, things improve daily.  Best wishes.

K-D Thank you so much for detailing the problems you have had with your multifocal implant. If I had not read your comments and those of a few other IOL patients, I would have been out of my mind with anxiety. I am experiencing many of the same symptoms and either the literature does not mention them (like ghosting)or sets the expectation that everything will be 'normal" in six weeks. I am only 5 weeks post surgery and it is only the rezoom lens in my right dominant eye that is the problem. My restor in the left eye is fine. Alhtough I'm experiencing halos and a lot of aberations in stores with fluorescent lights I am finding that the worst issue for me is ghosting. Anything beyond two feet is a problem and I haven't seen clearly in five weeks. It is far worse indoors and/or in low light. I cannot wear sunglasses at all as my pupil dilates and the symptoms worsen. To add to the mix I'm already beginning to have thickening of the posterior capsule in the rezoom eye so will require a YAG to correct. However, I will have to live with this until at least April as i'm out of the country next week for three months so the YAG will have to wait until April. My vision as of this week was 20/20 near and 20/25 distance but I expect my near vision which at least has been excellent in the rezoom eye, will deteriorate until I get the YAG done. I am hoping that as in your case with patience that over time the ghosting will gradually disappear as will the other less annoying aberations. I'm finding out slowly that what we have experienced seems to be common and do not quite understand why we were not advised of the probability of these occurances! At least knowing others have been there has helped tremendously. Thanks again.

You've probably already seen the posts by blue92 regarding ghosting issues with ReZoom.  But just in case you've missed them, please check out the recent response to "double vision after cataract surgery."  I have no idea whether minor lens decentration could be responsible for your ghosting problem.  But I do think that it would be a good idea for you to postpone a Yag procedure until after your ghosting problems have resolved.  From what I've read, an additional IOL procedure--just in case you need it--(e.g., recentering the lens, exchanging the lens) are much less safe/successful after Yag.

Thanks so much for the heads up on the decentralization problem with the Rezoom lens. I read the comments on double vision after cataract surgery on this board that you referred to in your post. Even though I saw doc last week (13 Dec) and was assured both lenses are centred perfectly I will go back again and ask to have a measurement done to absolutely confirm this. Many thanks also for the info on issues with YAG. I will definitely ensure that the ghosting is dealt with first.

tony - you have described perfectly the glare and reflection problem i am currently having. i received an AcrySof IQ fixed focus lens nov. 27, 2006. although my vision is improving daily after two months of poor vision and distortion, i still have the reflection problem in the corner of my eye. also, in soem types of light, i experience a jittery strobe effect that is very irritating. the black arc and heavy feeling are the most ittitating though. can you tell me if any of this has goen away for you? i have an appointment for a second opinion this wed. to see what options i have. it is so irritating, i would be willing to have it replaced with a different lens if that is an option. this particular lens has a double square backed design which can lend itself to this reflection problem, or so i read on the internet. glasses help, but he main reson i elected to have this surgery was to improve my vision and perhaps not have to wear glasses! let me hear from you please. thanks.

Has anyone posting had restor, rezoom, or crystalens IOL for 1 yr. or longer?  I must schedule cataract surgery and was advised by my surgeon that problems would likely develop with multifocals 1 yr. out. I currently wear glasses only for distance.  Take them off for intermediate tasks and reading. I'm considering Restore/ Rezoom combo, Crystalens, or monovision. Any thoughts on what would be best for my needed correction?  I use distant and intermediate vision
