I am 51 years old, had my first mammogram last week. They called back for a second look. My upper RB has a 1cm mass. Biopsy is recommended or wait 3 months for third look. Not a cyst, does not show on US.

Spot is round and dark in mammogram. Options are mammatome or complete removal using a more invasive wire device.

Any advice or idea as to what this might be would be appreciated. I have no family history of BC. I am prone to female disorders such as cysts and fibriod tumor.

What does complete removal using a wire device involve? What would you recommend in this situation.

Dear shopalong:  When a mammogram is done, it is usually given a BIRAD score which gives some indication of the level of suspicion and guidelines for follow-up care.  A BIRAD score of 1 means that the mammogram is "normal."  A BIRAD of 5 indicates high suspicion of cancer.  Without review of the mammogram and ultrasound and doing a physical examination, we cannot speculate as to what this might be or make specific recommendations.  You may benefit from a discussion with your doctor as to the level of suspicion seen in the mammogram.  This will also help to guide decisions as to what type of biopsy may be best if one is needed.  A mammatome biopsy is generally done in the radiology department while a wire localization procedure involves placement of a wire by the radiologist to indicate where the abnormality is followed by a surgical procedure.

I asked about the birad and was told it was 0. The radiologist simply advised a biopsy because he could not tell what the spot was. The surgeon we spoke to is going to run it by several radiologists to get their opinion before recommending any proceedure.

I've had a mamotome and a wire directed biopsy. The mamotome is less invasive. The wire-directed involves 2 proceedurees and thus is more of an ordeal. If they recommend the wire-directed, make sure the radiologist is going to numb you up well. (I guess some radiologists don't numb the area.) I was numbed well and mine wasn't too bad. If doctors felt the mamotome would be a reliable biopsy I would choose that. (My personal opinion only.) That's great that the doctor is going to get more opinions for you.

Thanks for the input. The surgeon told me that with the wire method there would be a complete removal. At first I thought I would want it removed by I'm not sure if the invasion is worth it. I guess I'll wait to hear from him this week.

Was yours benign?

I had a mammogram done and got a letter in the mail 55 days later saying they found a spot and they wanted me to come back in 6 months to have it checked (I belong to a large HMO).  I called to see if I could have it checked sooner, they said no.  Of course I have no idea what kind of spot, how large or anything.  I just get 'advice' nurses on the line.  I'm a bit worried as BC and tumors run in my family.  I fear that waiting 6 months for a return mammogram may be too late.  Or is it ok if the spot is really small (assuming they can't biopsy it or something).  Any advice would be great.

That is a dilemma! I would request a copy of the report so you have more information to work with. Without information you are left to 'trust' your doctors that 6 months is okay to wait. Likely, that is the case...though it's better to know definitively.
